Kaumātua Support
The service is provided as a day type programme with a prevention and intervention focus that is intended to improve the health and wellbeing of kuia and koroua Māori living in the Hutt Valley and who are 55 years and over.
Referrals to the service can come from a variety of sources:
- Self referral
- Whānau referral
- Assessment and service coordination service
- GP service
- Other health or social service provider
- Government agency
Other aspects of the service include:
- Weekly transport to rongoa and mirimiri clinic (when space permits)
- Transport to Kaumātua Kaunihera hui
- Ra Whakapakari Waiora days (health screening and assessment days)
- Education forums (workshops around topics of interest)
- 1:1 support and advice provided in the home
