ServicesOutreach Immunisation Service This service is designed to assist primary health care providers and well child providers by following up on Whanau who have tamariki that have missed their standard immunisations. The purpose of the service is to improve immunization rates amongst Maori, Pacific and other priority groups. The outreach service is staffed by a nurse vaccinator and a community health worker. Services are provided in the client’s home or other agreed venues.home. Referrals are accepted through primary health providers and well child providers, ie GP services. The coverage area for this service is the Hutt Valley and Wainuiomata.
