Outreach Immunisation Service

This service is designed to assist primary health care providers and well child providers by following up on Whanau who have tamariki that have missed their standard immunisations.

The purpose of the service is to improve immunization rates amongst Maori, Pacific and other priority groups.

The outreach service is staffed by a nurse vaccinator and a community health worker.  Services are provided in the client’s home or other agreed venues.home.

Referrals are accepted through primary health providers and well child providers, ie GP services.

The coverage area for this service is the Hutt Valley and Wainuiomata.

Outreach Meningococcal. B Immnunisation Service

This service was designed to assist primary health care providers and well child providers by following up on whanau who have tamariki who find it hard to access a general practitioner for the MENZB campaign.

This service is contracted to be provided from 5th May 2005 to 30 June 2006.

The outreach service is service by a nurse vaccinator and a community health worker services are provided in the clients home, marae, community settings and sporting venues.

Local events such as Te Ra O Te Raukura have been targeted. Radio campaigns have been organised, incentives such as tickets to live concerts, give-a-ways such as balls, bags, drink bottles.

Collaboration with maori and pacific providers have been some of the strategies we are or have used.
